Criminal Record Disclosure

Important notice for applicants who have criminal records: All applicants with criminal records, whether or not the crime is considered under Florida Statute 465.035, are required to complete and sign AMC’s Criminal Disclosure Agreement. If you plan to be licensed in the state of Florida you are advised to check with an attorney prior to enrolling at AMC because individuals who have committed crimes that fall within Florida Statute 465.035 are NOT ELIGIBLE for licensure in Florida. Other criminal offenses may be considered by the Florida Board of Acupuncture or Board of Massage Therapy on a case-by-case basis.
