1. An acknowledgement that the institution signed and returned to the Department the Certification and Agreement and the assurance that the institution has used the applicable amount of funds designated under the CRRSAA and ARP (a)(1) and (a)(4) programs to provide Emergency Financial Aid Grants to Students.

Acupuncture and Massage College (AMC) acknowledges it signed and returned to the U.S. Department of Education the Certification and Agreement on 04/12/2021, and has used all the funds designated under the CRRSAA and ARP (a)(1) and (a)(4) programs to provide Emergency Financial Aid Grants to Students.

  1. 2. The total amount of funds that the institution will receive or has received from the Department pursuant to the institution's Certification and Agreement for Emergency Financial Aid Grants to Students under the CRRSAA and ARP (a)(1) and (a)(4) programs.

The total amount of funds AMC received from the U.S. Department of Education for distribution to students is $150,663.00.

  1. 3. The total amount of Emergency Financial Aid Grants distributed to students under the CRRSAA and ARP (a)(1) and (a)(4) programs as of the date of submission (i.e., as of the initial report and every calendar quarter thereafter).

As of April 1, 2021 – June 30, 2021: $150,664.80

  1. 4. The estimated total number of students at the institution that are eligible to receive Emergency Financial Aid Grants to Students under the CRRSAA and ARP (a)(1) and (a)(4) programs.

The estimated total number of students at AMC that are eligible to receive Emergency Financial Aid Grants to Students under the CRRSAA and ARP (a)(1) and (a)(4) programs is: 105.

  1. 5. The total number of students who have received an Emergency Financial Aid Grant to students under the CRRSAA and ARP (a)(1) and (a)(4) programs.

The total number of students that have received an Emergency Financial Aid Grant to students under the CRRSAA and ARP (a)(1) and (a)(4) programs is: 105.

  1. 6. The method(s) used by the institution to determine which students receive Emergency Financial Aid Grants and how much they would receive under the CRRSAA and ARP (a)(1) and (a)(4) programs.

The methodology that was implemented for awarding the HEERF II grant amounts was the following:

a. A fixed amount of $639.00 (lowest full-time scheduled Pell Grant for the 2020-21 award year) was awarded to each of the 44 student applicants that were Pell Grant eligible/recipients for the 2020-2021 award year. ($28,116.00)

b. In addition, of the total 105 eligible CRRSAA HEERF II Grant applicants, a fixed amount of $202.56 was allocated for each of up to seven(7) different situations/scenarios that applied to each qualifying student as was marked off on his/her/their submitted AMC HEERF II (Spring 2021) Grant application. As a result, an additional $122,548.80 was disbursed amongst all 105 eligible students from 06/28/2021 thru 06/29/2021. The official breakdown of the CRRSAA HEERF II Grant amounts awarded are the following:

  1. i. Two (2) students were each awarded $607.68.
  2. ii. Five (5) students were each awarded $810.24.
  3. iii. One (1) student was awarded $841.56.
  4. iv. Seventeen (17) students were each awarded $1,012.80.
  5. v. Twenty-eight (28) students were each awarded $1,215.36.
  6. vi. Nine (9) students were each awarded $1,417.92.
  7. vii. Two (2) students were each awarded $1,449.24.
  8. viii. Seven (7) students were each awarded $1,651.80.
  9. ix. Nineteen (19) students were each awarded $1,854.36.
  10. x. Fifteen (15) students were each awarded $2,056.92.
  11. The method used for the 2nd round of funds awarded: The College awarded the 2nd round of funds to all applicants that met the eligibility requirements and submitted an application demonstrating need before the designated deadline.
  12. 7. Any instructions, directions, or guidance provided by the institution to students concerning the Emergency Financial Aid Grants.

All instructions, directions, or guidance provided by AMC to all students concerning the Emergency Financial Aid Grants have been sent via email & are published on the College website.

Students received the following instructions:

If you have received this notification, you may be eligible for an Emergency Relief Fund Grant as established by the Coronavirus Response and Relief Supplemental Appropriations Act (CRRSAA) 2021. You understand that these emergency financial aid grants for students may be used for any component of the student’s cost of attendance or for emergency costs that arise due to coronavirus, such as tuition, food, housing, health care (including mental health care) or childcare. You understand that the Secretary of Education does not consider these individual emergency financial aid grants to constitute Federal financial aid under Title IV of the Higher Education Act (HEA).

You may be deemed “eligible” for an emergency financial aid grant if both of the following are true:

1) You are eligible for Title IV Federal Financial Aid as evidenced by Acupuncture & Massage College having received a successfully processed 2020-2021 FAFSA/ISIR, whether you have received financial aid at our institution or not.

2) You are a degree-seeking or certificate-seeking Acupuncture & Massage College student in good academic standing that is actively enrolled in courses at half-time or greater enrollment status.

We are currently in the process of determining Spring 2021 grant amounts for our students. If you need an emergency financial aid grant you must:

  1. (1) fill out & submit the required application online at https://studentapplicationforemergencyaidspring2021.questionpro.com no later than 06/08/2021 and,
  2. (2) complete a 2020-2021 FAFSA no later than 06/04/2021 only if you have not already completed it previously.
